You may pick up CMHA Employment Application forms in the lobby at CMHA Central Office, 880 E 11th Ave, Columbus OH 43211-2771. Completed applications are due in the Human Resources department at the same address by noon on the deadline date. Please be sure to include the job number on all paperwork concerning your application.

Hours of operation

CMHA Hours of operation are: Monday through Friday, 8:00 a.m. to 4:30 p.m. except holidays.
